Before an appointment can be scheduled, a requisition form must be completed and signed by your primary care physician.
Ask your physician to complete and sign the requisition form at their office before contacting Clearview.
Download and complete the Clearview form digitally through our website, then have your physician sign it.

After your requisition is received, our team will review it for completeness, legibility, and approval.

Your requisition is approved by our administration team.
Once approved, we will reach out to schedule your appointment.
A Clearview team member will call you to find the day and time that works best for your schedule. Immediately after the call, you will receive an email confirming your finalized appointment date and time.
Pricing can be confirmed when you contact Clearview directly. Cost varies based on the procedure and whether you are paying out of pocket or using insurance coverage.
We currently accept BAF insurance. Please bring your physical insurance card so we can copy it for our records.
